ZIP Codes are usually unique to a particular city or county, but in rural areas, this is not always true. A single ZIP Code can span multiple counties but there is always a 'primary' county for the ZIP Code. A single ZIP Code can also be used for more than one city, but usually the additional cities are unincorporated.

For example, the US Postal Service confirms that ZIP Code 38583 is appropriate for Sparta, TN but it also acceptable for the nearby unincorporated town of Ravenscroft.

As of 2012, around 20% of ZIP Codes span more than one county (source greatdata.com).

Yes...if you use the term "city" loosely. In some rural areas, there might be a town or township that does not have its own post office and uses a neighboring post office, so they wouls share the same zip code.
