A butterfly koi tattoo can symbolize a great many things. A butterfly koi can symbolize faith, trust, hope, and love.
In all seriousness...NO! A koi fish, a lotus and a mythical phoenix bird should never be displayed together in a tattoo. This schmorgasborg, if you will, of magical and mythological items should never be displayed on ones body individually let alone cummulatively! There would be one scene that would perhaps justify the combination. Picture a mythical phoenix bird with bright red feathers being consumed by a vicious fire. Now picture this fantastical creature on top of a beautiful lotus flower symbolizing the relationship between the reborn soul and the love of a mother. Follow that by an emerald koi fish jumping over these phoenix consuming flames to symbolize the breaking free of our mind over the lusts of the body. Then and only then could you combine these three amazing things into a work of body art. ^^I get what this user is saying, but honestly, it's your body and your decision what goes on it. The whole point of a tattoo is to express yourself artistically. And if you want those 3 things, get them. Who cares what other people think.

I have an incomplete answer for this. I just got a tattoo of a koi fish that I designed (without the spot). I got it done in Japan and the tattoo artist decided to add it. He explained to me that it when a koi fish gets older...something something...and that it was very japanese. I has something to do with the aging or wisdom of a koi fish.
There are dozens of tattoo parlors located all across London, though they have different areas of expertise and some may not be able to tattoo a koi. Your best bet is to find a parlor near you, and then ask them over the phone if they are able to perform this specific tattoo.
