The proper way to add the suffix of a third generation name would be to simply add a space then three uppercase letters "I".
Examples:

Vincent Andre Paul IV
Some manuscript styles add a comma and a space after the last name. However, this appears to not be correct at all, unless someone specifically asks for their name to include the comma before the suffix.
