Yep, country people would simply dig a hole in the ground and cover it in when it got too full, the gentry would, if they thought about it, would have a wooden hut around it or a cesspit to throw the night's "soil" in but the aristocracy would probably have a guarderobe which was like a small cupboard that protruded from the main castle wall. The bottom of the cupboard was open to the wind and weather so it was probably pretty uncomfortable to use in the winter. There were also hooks fastened into the wall of the "cupboard" where their best clothes would be hung because it was believed that bad smells repelled not only insects but disease! Yuch!

Your economic position in society would have dictated the method you would have used for your toilet. Land owners and above who owned castles usually had a room like a closet that had a hole in the floor. it varied as to what they would sit upon. The eliminated product would fall directly into the moat surrounding the castle. No moat, well then they had to have a person to clear the area.

Yes, they had both public toilets and private ones in their homes. They were flush toilets too. The public latrines had a stream of running water to carry away waste, while the ones in private homes mostly used the "bucket-flush" method.

The overwhelming majority of the time, no they did not, nor did they have other forms of Plumbing. Water was generally drawn from a well, fountain, or nearby river or stream. Greywater was typically discarded outside, or in a town into a street gutter.

In a few very rare cases, some structures did have running water, but this was only in the case that a water source was at hand, and situated so that gravity could provided the force for the system. This was also limited to elite or expensive structures, such as a monastery. Typical people did not have running water in their homes.
