A knife was not often thrown. It doesn't make sense to take a weapon and throw it away.
They did use shuriken, either in the form of stars or small metal spikes, usually to distract the enemy, they were too small to do the sort of damage you see in the movies.
It would differ because the blade spins when thrown. Usually they threw it by the blade, but each person is different. They would adjust how hard to throw it based on the distance to be covered.

Contrary to the above, a ninja would use whatever was at his disposal to eliminate a threat. Knives are sharp, can be used to cut stuff (hence sharp), look nice, and you carry them around, if you feel like it. They did use shuriken indeed, although there are more types of shuriken than the commonly known "ninja star", or "Hira Shuriken", and among them includes "Bo Shuriken", which is basically a sharp metal stick used for throwing. This is very much like a knife, so I see no reason why they would not use one.
(Also, sometimes shuriken would be poisoned, sometimes by placing it in a pile of animal manure so they would rust. The rust would harbor Clostridium tetani, which, when penetrated deep enough into a target, would cause the enemy to get tetanus. Though this is curable now, back then, this was incurable and lead to death.)

About throwing the knives by the blades or by the handles, it doesn't depend as much on the way the ninja wants to throw it, but it depends on the distance. Using a proper form, the blade would typically, if balanced correctly, do a half spin every 3 feet (approximately). Using this knowledge, if an opponent was 15 feet away, the blade would spin 2 1/2 times (15 / 3 * .5). From this, because you want the blade to be pointing into the enemy, you would hold the blade, so that when released, the blade would change directions, into the target. Obviously the ninja would not whip out a handy ninja measuring tape that was so common at the time (it wasn't), it was mainly a matter of guesswork.

Depends entirely on the knife. Most of the time people try to throw knives that have a heavier handle than a blade and that's just stupid... The best throwing knives are knives that have a heavier blade than a handle for example a Bowe Knife. But, if you have a knife like the one I described the trick to throwing it is to hold it by the tip of the blade and then aim at your target.
