yes he did die in typical jedi fashion...he wasn't killed though. he died of old age. in the final installment of the Starwars trilogy, Luke is with him in his home when he fades away into nothingness, joining Obi Won on the other side. They were both later joined by Anakin, Luke and Leia's father. Obi Won and Anakin did a "crossing over" of sorts, although since they are no longer among the living, the are thought o have "died".

In 5 and 6, Yoda was a puppet. In 1,2, and 3, Yoda was animated (computer animated.) He was also animated in the Clone Wars(which was obvious)

Of course all of these calculations are based on the belief that Yoda was only days or months away from his 900th birthday before his death; its actually impossible to know his exact age because George Lucas hasn't revealed that information. Still, this is as good as it gets without actually knowing Yoda died at exactly 900 years of age. In Star Wars episode 1, he was 865.
