Under FMLA, nothing qualifies for additional time after you have used your initial 12 weeks in the defined 12 month period. You may be able to exercise your ADA right to a reasonable accommodation for additional unpaid leave but it would be unusual. You might wish to consider consulting an employment attorney or investigating your options with disability insurance.
You should also discuss the situation with your employer, as although they are not compelled by FMLA to offer you an extension, they may willingly do so.
