Paranoia...schizophrenia in general would be verbal confusion - anything that makes them difficult to understand that isn't readily explained by something else. A stutter probably wouldn't be an indicator but rambling...think homeless man on the street. Being a shut-in, having fewer friends, less and less socializing. Franticness or anxiety about society in general. And of course any sort of ideology that people are out to get them! If it begins to impact their life in a negative manner - they need to seek help.
