Your best bet is to take the car to a professional detail shop with a commercial steam cleaner. Home use steam cleaners for upholstery just don't have the same effect as a commercial machine. Additionally the detail guys deal with this equipment on a daily basis, and know how to get the best use of it.
Additionally a detail shop should have a high power O-zone generator that can be run inside the car for a day or two to neutralize bad odors.
