It is awarded for "Conspicuous gallantry and intrepidity at the risk of his or her life above and beyond the call of duty while engaged in an action against any enemy of the United States; while engaged in military operations involving conflict with an opposing foreign force; or while serving with friendly foreign forces engaged in an armed conflict against an opposing armed force in which the United States is not a belligerent party."

Due to the nature of its criteria, it is often awarded posthumously.

Because Medal of Honor recipients are few in number, the selectivity of the medal, and the fact that it is awarded by the President of the US, in the name of the Congress, the President, as Commander in Chief of the US Armed Forces, makes the award. If unforeseen circumstances make the President incapable of making the award personally, his designate would make it.

They are nominated by their chain of command for heroism well above and beyond the call of duty. If it is approved through the entire chain of command, the award will go to Congress for their approval. If they approve the medal, it will be awarded.

The official description is, performed conspicuous gallantry and intrepidity at the risk of his or her life above and beyond the call of duty while engaged in an action against an enemy of the United States.

Let's say you were a member of a self-propelled artillery section that came under heavy counterbattery fire--which means the enemy has figured out where you are and is shelling you with their own artillery. After getting the cannon crews under cover, you run out into the shelling numerous times to pull wounded members of your ammo team under cover. You are repeatedly wounded, but refuse treatment until all your comrades are taken care of.

This is the kind of thing that MIGHT get you the Medal of Honor; it also might get you any lower award right down to a DA Certificate of Achievement depending on how bad your commander hates you. The Medal is very political; the same act will get you recommended for the Medal of Honor in one outfit and not in another.
