Depending on the type engine there are different answers:

Every engine:
  • Head gasket failure, from over heat or warp-age or just failed
  • Some auto engines have the pump mounted on the timing cover so the gasket can fail and pump coolant into the oil
  • Many V configuration engines can have an intake gasket failure and cause this
  • A casting "porosity" meaning the metal itself can leak
Heavy Diesel And gasoline engines:
  • All of the above
  • These engine have an oil cooler that takes coolant and runs it through a "radiator" with oil to cool the oil. As they are Bolted together they can leak oil into coolant and coolant into oil.
