Some people choose to run their PHP files via the .phtml extension. Web servers, such as apache, allow us to define our own extensions. For apache, you'll have to load httpd.conf and find/edit a line in similar fashion to : AddType application/x-httpd-php .php .php3 .phtml .foo : It means that PHP will parse .php .php3 .phtml and .foo files on this server.

PHP is a programming language. Unless 3D max has a programming interface of some kind, you can't open php files with it. You can open a PHP file with notepad or any other text editor.
A few methods here. Firstly, if you have a cPanel or if the Control Panel of your provider has a file directory system you should be able to edit PHP straight in the browser. cPanel offers this with even syntax highlighting. Secondly, Notepad and FTP. Make an FTP account (or use an existing one) and download the files and then upload to new PHP files. You can download an FTP client free such as Core FTP. Thirdly, FTP online. Using such websites as online-php.com you can connect via FTP to your server and edit PHP files. It's a very nice program (even syntax highlights although this can make it slow to use) especially if you do not have the permissions to install an application version of FTP or if it's a shared computer. Last and my favorite method, Dreamweaver CS5 (unsure if CS4 has it). You can connect to your server via FTP. Connect it and sync the files on the server to a folder on your computer. Dreamweaver offers a layout designer, Tag suggest menu (when you type a letter it finds tags quicker -like something you would find in visual studios) and full coding support.
