In order to change your name on your Social Security card in Kentucky, you will need your divorce decree or a certified copy and your marriage certificate or a certified copy. Or, you can ask that your name be changed back to your maiden name during the divorce, which will be written into the divorce, and take that to the Social Security office.

After your name is changed on your Social Security card, you can get your license in that name. The name on the drivers license has to match the one that is on the Social Security card.

State laws can vary, but it is highly likely that these are the requirements in other states, too.

Same as anywhere. Show your divorce papers to the Social Security office and get a new card. Then go to the DMV and do the same thing. Notify your utilities provider, your bank, etc.

You should visit your local family court and inquire about filing a petition for a name change. You could also check your divorce file to see if you were given that right at the time of the divorce. If so you can start using it with no further legal action. In either case you should send official notification of any name change to the Social Security Administration and DMV and to every entity that has you in their records under your to-be-abandoned name: banks, employers, insurance companies, doctors, credit card companies, schools, etc. You could also have a new deed drafted by an attorney if you own real property.

Go to your local Clerk's office or the courthouse where the decree was filed; as to file a "Renunciation of Name". Also, check and see if your attorney made that part of the divorce decree.

This may vary by state, but as long as no fraud is involved, whatever name you want to use is pretty much up to you. Inform your DMV and bank. If more is required they will let you know.
