The Adam's apple appears when the thyroid cartilage in your neck increases in size. This occurs due to the effects of testosterone, and is associated with male puberty. The thyroid cartilage increases in size to allow the vocal cords underneath to lengthen, which is what allows men to develop a deeper voice than women or children.

Males will always have an Adams apple. When they are babies they aren't visble until you hit puberty!
