There are two ways to get to Rikers Island on public transportation: the Q100 bus, and the Rikers Island van. If you're starting out on the D train, then it's easier to take the van.

You can take the D train to 161st Street-Yankee Stadium, and transfer there to the Downtown (Manhattan-bound) 4 train. Take the 4 to 149th Street-Grand Concourse.

The Rikers Island van departs from 149th Street between Anthony J. Griffin Place and Park Avenue. You will exit the subway at 149th Street and Grand Concourse. Walk 1.5 blocks east to 149th Street between Anthony J. Griffin Place and Park Avenue.

The D train skips the 161st Street-Yankee Stadium stop during rush hours in peak directions. On weekdays, the Downtown (Manhattan-bound) D will not stop at Yankee Stadium between 6:15 and 9 AM, and the Uptown (Bronx-bound) D will not stop there between 4 and 6:45 PM.

When the D is not stopping at Yankee Stadium, the B stops there instead. In Manhattan, you can transfer from the D to the B at 145th Street. In the Bronx, you can transfer from the D to the B at Tremont Avenue.
