I personally find it really useful to jog a little ways slowly then stretch well after working out. And sometimes when I'm really sore in an area i will ice it for twenty minutes. But every so often I just seem to be sore everywhere, so then I take some ibuprofen and then hop in an ice bath.
Yes, along with stretching exercises. It helps stretch out your muscle.
It also helps to speed up the process of removing the waste lactic acid from your muscles, which stops the risk of cramps later on.
After you exercise, your body will already be warm, but you may be perspiring and your skin may become cold. Wrap yourself with loose towels or clothing to keep warmer air next to your skin. Becoming chilled after exercise is not healthy.
I usually cool down with the same routine as I warm up with, then follow it with a stretch. This routine includes high knees, butt kicks, lunges, sumos, opening and closing the gate, etc. Another good cool down is a bike ride for about 30 mins, followed by stretching or yoga. Rollers are great for when you are stretching to get all of the lactic acid out of your muscles/ stop it from building up.

Gentle exercise in it'self is an excellent way to warm up.
