It can climb another 5-10°, so you should "undercook by a few degrees", and worst case would be you'd throw it back in. You can't save an overcooked piece of meat. Also, do not forget to let the juices settle by leaving the steak untouched for 5 minutes.

The color of the steak under the outside sear will be gray. The steak will cook till there is just a narrow band of pink in the middle. Medium rare is considered too done by many fine dining chefs for it masks the flavor of the meat. If you or your guests likes your steaks or burgers cooked medium well than do what makes you and your guests happy.

There are too many variables to answer your question succinctly. The variables are: Thickness of the steak, temperature of the grill and distance of the grill from the flame. For a 1-inch steak, try using a medium heat and flip the steak after 5 minutes. Give the steak another 3 minutes and cut into it. If the center is not pink, you over-cooked it. If it is red, you didn't cook it long enough. Great thing is, if you have a lot of people at your grill-out, somebody will like that steak and you can adjust your cooking time accordingly for the rest.

A medium rare steak should be warm throughout the middle and from 130 to 135 degrees F. (55 to 57 degrees C.) The center will be mostly pink, and juices will be reddish brown and should flow copiously when the steak is cut.
