It depends on the thickness. The USDA recommends cooking pork until the temperature in the center (the coldest part) reaches at least 160 degrees Fahrenheit. It should be obvious that that's going to take longer for a really big thick piece than for a smaller thinner piece.
If it's about 3-4 lbs in a crock pot it will take about 5 hours if cooked on low. You should also cook fat side up.

It is not necessary to do so. Depending on how big the pork roast is, you may want to put foil over it for the first 3/4 of the cooking time (to prevent dryness), removing the foil for the last 15 minutes or so, just to brown the meat.
