The honest answer is that they have absolutely no idea. The 'deaths' are generated by computer models - GIGO, Garbage In, Garbage Out! The models are based on unreliable, inaccurate, imprecise, out-dated, "Policy Based, Evidence Making" epidemiological studies. By cherry picking studies and the 'creative' interpretation of their results (often flaunting the fundamental principles of epidemiology and statistics in the process), any required number may be obtained.
Similar GIGO models are used to generate the number of "obesity related" and "alcohol related" 'deaths'.
