Quarantine has been used to contain diseases. For example, if a hospital gets a case of SARS, which is a very contagious respiratory disease, they'll go on quarantine. No one in, no one out until it's dealt with.

Quarantine can be applied at home too, but it basically just means, keeping the sick person in your house so they can't give their cold to anyone else. That's why several businesses recommend that their employees stay home when they're sick. It isn't out of interest for the employee's well being, it's because they don't want the employee getting everyone else sick!

Also, good hygiene. Wash hands, keep yourself clean. Kill germs that way they can't spread.

Basically, just apply common sense.

Now, when the disease is a serious one, like Ebola, which is a viral hemorrhagic fever, the CDC would probably get involved. I'm not going to go into that too much, because I'm guessing, hoping, that you don't have to deal with Ebola. And if you do deal with Ebola, you should know how to deal with it and not have to ask the question on this site!

You have to drink safe or potable water. That way only you can save yourself from water borne disease. The easiest way is to carry your pack of hypochlorite solution with you. You can add a drop to four litre of clean water and then drink the same about one hour later.

What is one of the most effective ways to prevent catching a communicable disease?

You need to have prevention at three levels. First is that, the disease should not be transmitted from the source. Secondly the you need to control the disease at the transmission level. Here you have to use the barriers like mask, supply of safe water, not sharing the drinking utensils and control of the vectors etc. Thirdly you have to protect the recipient by vaccination, by passive immunity, by personal hygiene and protection from the vectors.
