If the spider bite resulted from the work assignment or occurred in the course of work, and if its treatment required more that First aid, it is OSHA recordable, regardless of whether work was missed or not.

If prescription medication is required as a result of the sting, then it is recordable. OSHA does have an exception for tetanus shots given as a precautionary meaure after a puncture, however, these are rerely given for insect bites or stings.

An insect bit may be OSHA recordable if it occurs as a result of you performing your work assignment and you are hospitalized or loose a day or more from work as a result, or you receive medical attention beyond first aid for the bite.
