A disease is a condition that causes illness, it could be an infectious disease (like pneumonia, mumps, or a wound infection), or it could be a disease caused by a disorder of organs, genetics, hormones, or wear and tear on functional body parts, etc. (such as Diabetes mellitus, Heart disease, Arthritis, etc.).

A symptom is what the person with the disease experiences as a consequence of the disease from the patient perspective. Examples are: pain, nausea, dizziness, fatigue, bad taste in the mouth, blurred vision, etc. It is a manifestation of the disorder that is subjective and must be reported by the patient, since only they experience it.

A symptom differs from a sign, which is what health care professionals can see and evaluate from an objective view. Things such as measurable vital signs (e.g., fever, high blood pressure, fast pulse), or observable effects of disease or injury (e.g., red and irritated-appearing skin, perspiration, a sore, a broken bone, a collection of pus, etc.), or objective test results (e.g., reports from laboratory testing, X-ray images, what a doctor/nurse feels with her/his hands on examination, etc.), or other physical examination results like nervous system reflex responses, pupil reaction to light, what your pulse feels like or your lungs sound like.

A sign is an objective observation, such as the temperature of the person. If that temperature is above normal we call it a fever.
