Diseases caused by pathogens are often contagious and include things such as Meningitis, Influenza and Tuberculosis.

Diseases not caused by pathogens are generally things which go wrong in the body. For example, Cancer, Multiple Sclerosis and Motor Neurone Disease are all diseases not caused by pathogens.

Yes, all diseases are caused by pathogens. The only difference with a chronic disease is that it lasts much longer than an acute disease.

Pathogens make up the entirety of things that cause disease, whether they be viruses, bacterias, or other organism. It doesn't matter if it's a chronic disease or not.
