once, oakland
Answer--this has actually happened quite often. When a team is losing bad AND/or it has exhausted its pitching staff during the game, it will use position players to pitch. I remember seeing something on sports center about an outfielder who had pitched three or four times in the last three years in the above situation, He threw barely 65 mph compared to the standard 85-95mph of a MLB starting pitcher, but had a great curve and was hitless !
yes they can be replaced at any time
Yes, a pitcher may be replaced at any time with one exception. That exception isifthe pitcher has just entered the game and the batter is the first batter he is pitching to. The rule states a pitcher must pitch one full at bat and applies to both starting and relief pitchers. Of course, if the pitcher suffers an injury the umpires can rule that he may be replaced regardless of whether the batter is the first batter the pitcher is pitching to.
Pitcher is noun.Pitch is a verb. Forms are - pitch / pitches / pitched / pitching
A pitcher who enters the game must pitch at least one full at bat. The only exception to this is if the pitcher suffers an injury during warmups or while pitching to the first batter that the umpires deem serious enough to warrant medical attention. Then the pitcher may be replaced before pitching one full at bat.

The job of a baseball coach is to make the right decisions for the team. For example if the pitcher is not pitching well then the coach will take the pitcher out a put in a new one.
His/her coach would take the pitcher out and have a doctor look at him/her. Then, a sub would be replaced.
