It more than likely needs new bearings. That is why it is out of line, unless the vehicle has been in an accident. Remove the belt, and remove the tensioner idler pulley and repair it.

The idler will have a smooth surface but the tensioner will have grooves on the pulley & should below the idler. Most likely you need to look for a grooved pulley that looks like it may not be related to any parts of the engine,(i.e. the water pump pulley, a/c pulley) this is the tensioner pulley. The tensioner pulley on my '97 GMC 5.7L is located just beneath the alternator. Inorder to relieve the pressure from the "tension arm" find a square opening molded into the pulley arm, and insert a breaker bar to use as a lever for relieving the tension. Most vehicles will have a diagram in the engine compartment showing the specific route for the belt to go on the pulleys. There should be one on a panel under the hood of your car. Typically it sits above your radiator or in that area. If not then look for the tensioner pulley. If so take a rench and see if the pulley moves up or down, or side-to-side when you apply pressure to the wrench. If it does, that is your tensioner. Simply get your belt started on all of the other pulleys and then apply pressure to the tensioner pulley until it is positioned in such away that allows you to slide the serpentine belt onto it.
