It depends on your tolerance to pain. Even if your cavity is small, if the drill or polisher gets near the nerve, its going to send shocking pain. I had a very deep cavity that I had filled and had mistakenly taken quite a bit of Vitamin C before the procedure. Vitamin C will prevent Novocaine from working. It was very painful, but at my threshold.

On the other hand, Novocaine is pretty shocking to the body and to the immune system itself. If you're thinking of forgoing it for health reasons, and the cavity is small, it might be worth it to give it a try. You can always ask for Novocaine halfway through if you need it.
