Rupture typically occurs in males while bench pressing heavy weights, with the shoulder extended and abducted. Excess tension with the muscle fully contracted can then cause either partial tear or rupture. Rupture is more common than a partial tear, and is most commonly seen along the sterna border. Rupture is usually associated with avulsion as well. Injury can be classified based upon whether it is a complete or partial tear, and it's location (sterna vs clavicular vs bicipital groove). A significant risk factor is the use of steroids as increased muscle strength occurs without associate increase in tendon strength.Immediate treatment includes immobilization of the limb, ice, and referral to orthopedics. Physical exam depends upon location of rupture but is associated with bruising and mass in the muscle belly. Gold standard for diagnosis is MRI, but ultrasound now offers a more cost effective form of imaging. Long term treatment is surgical with significant post-operative physical therapy to help remobilize muscle and avoid scar tissue. All of this electricity is generated and directed by the cardiac electrical system.The heart produces electricity due to ions of calcium, potassium and sodium entering and exiting heart muscle cells. This is known as the sodium-potassium pump. Normally, sodium is located outside the body and potassium is located inside the body; when these ions switch places, the body uses active transport to return potassium to the inside of the cell and move sodium outside the cell. When electricity is generated, it passes through the SA node. The SA node is known as the body's "natural pacemaker" as it sets the rate at which this electricity is released. The amount of electricity released is proportional to the heart rate. During times of rest, the SA node releases electricity that causes the heart rate to occur somewhere between 60 and 100 beats per minute. The SA node releases electricity into the cells of the atria.The atrioventricular (AV) node is located at a point between the atria and ventricles. Upon receiving electricity from the SA node and atria, the AV node regulates the electricity into the Bundle of His. The AV node also contains cells that are able to create electricity. That said, the amount of electricity created by the AV node is less than the amount that is created in the SA node. In the event of an SA node malfunction, the AV node is capable of serving as a "backup pacemaker." A heart controlled by the AV node is capable of beating 40-60 times a minute.The bundle of His transfers electricity from the AV node into the ventricles. This electricity is transferred from the bundle of His into several branching fibers known as the Purkinje fibers. These fibers branch of into different portions of the ventricles. The release of this electricity causes ventricular contraction. The bundle of His is also capable of generating electrical current in the event of SA and AV node failure. A heart controlled by the bundle of His is capable of beating 20-40 times per minute.Electricity is generated in the SA node via the sodium-potassium pump. From there, electricity passes through the atria and into the AV node. Electricity passing through the atria on the way to the AV node causes the atria to contract. The AV node transfers electricity to the bundle of His and the Purkinje fibers. This causes ventricular contraction. The hearts electrical system stimulates the cells in the heart to beat.The depolarization cycle of the heart is mapped by an electrocardiogram. The P wave represents the SA node, the PR interval represents the AV node, the QRS wave represents the Purkinje fibers and the ST segment represents the repolarization of the ventricles. Medical personnel use the waveform created by the ECG to diagnosis cardiac electrical conduction problems by comparing the waves to the normal standard. (MORE)
