It depends which movie and, for that matter, which book. There have been at least a dozen movies made of Romeo and Juliet, more if you count the ballets and operas. Shakespeare's play wasn't the first telling of the story: he got it from Arthur Brooke, who got it from a French story, the author of which got it from an Italian story based on earlier Italian stories which are based on the Latin. Or you could be talking about an actual book which was based on Shakespeare's play, which is not strictly speaking a book.
You need to specify which movie. There are at least four cinematic treatments of the play.
There have been countless film adaptations of Romeo & Juliet. It's impossible to know which one you're referring to. Please ask another question and be more specific.
In both the play and the movie, it is the nurse who raised Juliet, who carries the wedding information between Romeo and Juliet.
There is no character by this name in Romeo and Juliet.
The main difference is the amount of dialogue which is cut. People these days don't want long, boring speeches, so they decided to cut out most of them. This is replaced with mostly kissing ( in the romantic scenes between Romeo and Juliet) or nothing.

Depends which "new and old" movies you are talking about. Significant movies have been made of Romeo and Juliet in 1936, 1955, 1968, 1996 and 2013. In the 1936 movie all of the actors are middle-aged, including Romeo and Juliet; in the others Romeo and Juliet are much younger. In the 1996 movie the settings and costumes are all about 90s California, whereas the 1955, 1968 and 2013 ones are definitely Italian Renaissance. The 1955 movie has a lot of dialogue cut out, as does the 1996 movie, but the 2013 movie has new dialogue written more or less in Shakespeare's style (but inferior).
Craig Pearce and Baz Luhrman wrote the screenplay for the movie, Romeo + Juliet (1996)
The Romeo and Juliet movie starring Leonardo DiCaprio was produced in 1996.
