Pure chocolate comes from Cocoa beans...a typical chocolate bar will also have sugar, milk (if it's Milk Chocolate, not if it's dark), cocoa butter, lecithin, flavorings (like vanilla) , sometimes vegetable oil, and a few minor ingredients for texture.

You need to include all of the essential ingredients!!!!!!!!!!!!! Don't forget the wax.

Hot Chocolate definitely contains some version of cocoa (either as natural cocoa powder, Dark Chocolate, milk chocolate or even white chocolate, the latter only containing cocoa butter). It also has to contain some kind of a liquid. The most common is milk or cream, but I recommend water as the healthiest substance. It is also possible to make hot chocolate based on tea or even mulled wine. Most common additions are vanilla, cinnamon, chilli, mint (candy), liqeur. Again, you can get creative and add marzipan, jams, banana...
