I've read the play, and as far as I could tell, it never says their given names, but if you want to refer to them, call them the above names (their last names).
We only know their surname, Montague. They are referred to as "Montague" and "Lady Montague" in the text. They do not have given names. The 1996 Baz Luhrmann movie made up names for them: Ted and Caroline.
