Obviously pain. The toe doesn't necessarily look "different" than the others (discoloration, swelling, etc) From past experience, the only way to know for sure if its broken is to get an x-ray.

An obvious sign would be a protruding bone coming through the skin. Another sign would be a deformity on a limb. Less obvious signs would be pain and swelling at the site of the break. A hairline fracture of the leg may hurt when the person walks on it, but not otherwise.
