I have this same exact problem and every test in the book. Mine is from L5 nerve impingement from an injury and this has started to now cause sever buttock pain along with the pain radiating into my hip and legs. The groin pain can be radiating from the L5 nerve or it could be impingement at L3-4. Any nerve impingement can cause this whether its from stenosis, herniated disc or spinal problems like scoliosis.

Look into a torn hip labrum diagnosis. Doctors are grossly underaware of this now common diagnosis among younger, athletic folks with hip pain that begins as a severe tightness in the groin. Based on lessons I learned, I strongly suggest seeing an orthopedic hip specialist first. "Back doctors" seem unaware of the torn labrum diagnosis and rely on their own pet theories. It took me 1.5 years of trying to get properly diagnosed.

When groin pain occurs in an individual, the cause can greatly vary between women and men; for instance, conditions affecting genitalia are likely to contribute to the pain. While men suffer groin pain due to underlying problems with male genitalia, women could have a few potential causes which are not related to gynecological reasons whatsoever.
