I am assuming you are referring to a standard transmission 1)could be a foreign object in there somewhere 2)could be seized you might want to open it up and take a look? I do not know if I am spelling this right, but it sounds like the synchronizers are bad. if it is a manual one of the shifting forks in the trany may be broke did that on a tractor

The toe and heel shifter pegs are for shifting up and down. Step on the toe shifter and it moves your gears to the lowest gear. Step on the hell shifter and you will raise the gears as you pick up speed down the roadway. Toe down and Heel up for gears.
