What could be the problem when a 1991 Mercury Capri-when first starting runs terribly till it shifts to second then runs well. You advanced the timing and it ran great then it did it again?

Check your vacuum during this, I believe this vehicle uses vacuum to determine engine load and thus shift points. If you have a medium vacuum leak it will shift funny unless the throttle is part open and even then it can be a little harsh.
