Three types of health care professionals in the surgical unit include the surgeon, surgical assistant, and the holding area nurse. When a physician has specialized in the surgical field, they are called surgeons. The surgeon meets up with the patient to evaluate them and schedule a day for their surgery. The surgeon will then do the interventional procedure and he will then take care of the patient until they are discharged from the hospital.

Because the surgeon cannot perform the whole procedure all by themselves, they have the surgical assistant assist in the surgical procedure. The surgical assistant will gown and glove like the surgeon and help him/her with the procedure. This includes retracting tissue in order for the surgeon to clearly see what they are working on, helping with hemostasis of the patient, and sometimes closes the patient using different suturing techniques.

Finally, the holding area nurse is a special type of nurse in the surgical unit. They are either a registered nurse, or a licensed vocational nurse. When a patient is being admitted into the surgery floor, the holding area nurse is responsible for receiving them, and documents their health status. They are also responsible for starting the patients IV and to thoroughly explain to them the OR environment. Apart from this, they are the bridge that connects the OR area and the family. These nurses are the ones who interact with the family of the patient the most.

The surgical unit provides the hospital with various surgical procedures. The purpose of this department is to treat the patient's diagnostic through surgery. In this unit, in order to perform surgical procedures, there is a set of healthcare workers that work together in order to successfully perform the process. Before any surgical procedure is performed on the patient, there must be preoperative care. This is where they identify the physiological, psychological, and sociological needs for the patient in order for the surgery unit to plan ahead for the surgery preparation. In the OR, there is certain OR attire that you must wear in order to prevent infection to the patient and cause complications. If you are just assisting in the OR room such as a scrub nurse, then you must wear a head cover, scrubs provided by the hospital, foot covers, and a mask. If you are actually a surgeon or an assistant, then proper sterile gowning must be done. This is where the person is gowned and in sterile materials in order to prevent contamination to the patient. During the procedure, proper OR etiquette must be used. This is where you respect and are aware of the sterile field. There are general rules such as, "sterile only touches sterile" and "if in doubt throw it away". Once the procedure is done by the surgeon, the surgeon hopes that the procedure will help the patient fully or help the absence of their disease.

3- Give one example of a legal issue that is relevant to this unit. Elaborate on the issue and use the legal term (i.e. battery, libel/slander, malpractice) related to this issue.

One example of a legal issue that is relevant in the surgery unit is malpractice. For example, one of the most common mistakes that is seen in hospitals is performing procedures on the wrong patients because the health care workers do not check the name tags. The RN in the surgery unit is in charge of moving patients from the surgery waiting room to the OR room and basically keeping up with the patient chart. If there is a procedure that will be performed and the RN never verifies the patient chart with their wristband and the surgeon performs a leg amputation on the wrong patient, malpractice is taking place. Malpractice could also include just performing the procedure on the wrong leg or arm of the patient. In order to prevent this malpractice, the RN or any other health care employee who is transporting the patient must check the wrist band and make sure it is the same to the chart. When the patient is on the OR bed, they must also re-check the wristband, chart, and make sure that they are performing the right procedure on the patient.

Because many times you are dealing with delicate procedures in the OR room, standard precautions are greatly enforced in the surgery unit. Because surgeries take place in the OR rooms, sterile standard precautions are used in the unit. Health care workers use the foot covers, head covers, scrub provided by the hospital, and mouth covers when on the Surgery floor. When dealing with patients, such as preparing them for a procedure, you wear gloves to protect yourself and the patient from any type of infection. Before the doctor arrives for the procedure, the RNs and other health care staff prepare everything that is needed for the procedure. One thing that is prepared is the sterile table with the sterile utensils that will be used during surgery. Before setting up the table, the health care worker must use sterile gowns and sterile gloves. Because they are setting up a sterile environment, sterile rules are applied to the preparation such as anything sterile touches only other sterile items, and everything below your sterile field and any sterile items you turn your back on are considered contaminated. Once the sterile item touches another item that is not sterile, it is considered contaminated and must be properly disposed of. Before the sterile preparation is set up or other set ups take place, the worker must wash their hands before and after. After the procedures are done, and the patient is in the post surgery recovery room, disinfection of the room is required. This means that everything that was used for the procedure is disposed and chemicals such as chlorine are used to disinfect the floor. Another standard precaution that must be taken under consideration in the OR room is if you are not in a sterile gown, then you must not touch anything that is covered in blue. If it is touched and you are not sterile, then the whole set up that was touched must be redone and lots of money is wasted.

There are many ways that a health care worker might be exposed to blood/body fluids or OPIM in the Surgery Unit. Because mainly all of the procedures involve inserting instruments and opening up the patient, bleeding is always occurring and could get pretty messy if not attended to. Many instruments, gloves, gowns, and cloths are covered in blood and if not careful they can go onto the health care workers skin. If a surgeon is not wearing a face shield mask and blood is splattered onto his eye, they could be contaminated with blood. The eye becomes a portal of entry through the mucous membranes thus contaminating his body with possible diseases. Another way that a worker could be exposed to fluids is if a body fluid is squirted onto skin of a health care worker and they have a portal of entry such as a cut, the fluid could infect the health care worker
