"¿Cómo estás" means "How are you?" (informal)

"¿Cómo está usted?" means "How are you?" (formal)

Strictly speaking, both the above should have the accents shown in the first example. Without them,

'como estas (niñas)' can mean

'like these (feminine-gender objects)

Me como éstas ---'I eat these ones (feminine-gender objects)'

NB 'Feminine-gender' is phonetical-phonologically important and that's why you must be careful with accents in Spanish, for it is not the same pronunciation of:

Éstas ['es.tas] (These ones, omitting the noun in fem. gender) / estas ['es.tas] (These, after or before the noun in fem, gender)

In comparison with:

Estás [es.'tas] --- You are (informal singular)

In English, "como estas" means "how are you?" A great response to this question would be "soy bueno" which means "I am good" in English.
