Heroin, unlike alcohol, does not destroy the liver and so on. Heroin becomes dangerous through how it is ingested and what it is cut with.

In countries with legal clinics where addicts can use, levels of HIV, DVT and 'needle related' illness / injury and infections dissipate because addicts have clean, available and controlled access to clean aparatus and surroundings...unlike addicts in countries in which heroin is illegal.

Overdose is most commonly caused due to the varying potency of 'street' heroin. Unlike alcohol for example which comes with a label telling you how strong it is, heroin bought illegally is neither pure, nor labelled. So an addict prepares a full bag or score =, yet it may be ten times stronger than the last bag they bought.

As well, 'cotton fever' and 'dirty hits' occassionally circulate in batches of heroin or are attributed to fibres and parasites within the types of cotton buds or filters used to draw heroin through. Both cotton fever and dirty hits propel the addict into a high fever and severe flu like symptoms, even hallucinations.

There are a lot of health problems caused by an addicts lifestyle, the legality of heroin and how heroin is used, but these are indirect...heroin itself causes few issues.

For example, a lot of addicts in countries where heroin is illegal are malnurished because of a combination of factors attributed or caused by the lifestyle which an addict has to adopt to feed their habit.

Heroin can indeed kill you. The process, with unadulterated heroin, is respiratory suppression and collapse typically after unconsciousness. With street heroin, other problems come into play. Notably, there is huge variance in dosage. Injected, the amount of heroin equivalent to a grain of rice, can be very strong indeed and potentially lethal, if the concentration is high enough. More, heroin is often mixed with other substances, in order to reduce cost. Depending on what the adulterant(s) is/are, a myriad of reactions are possible from allergy (from nearly any organic) to outright poisoning (very rarely, heroin has been cut with strychnine). Heroin is not typically lethal and in fact is more often comparatively benign (say, compared to methadrine), but is still very powerful in pure form, and adulterants make it an entirely mixed bag of possible effects. Depends on what you mean by "bad." The drug itself, like virtually any other substance, can cause an overdose. The problem with heroin is that, because it is illegal, there is no control over purity or dose. You might purchase a twenty-dollar bag today that is ten-percent heroin, but the one you buy tomorrow may be eighty-percent heroin. Since few users have the means to test the product themselves, except by taking it, well, you can see what the problem is.

Opiates themselves are among the most thoroughly studied drugs known. They have been shown to be completely safe, taken in proper dosages. Opiates cause no organic damage. They can be taken indefinitely without causing any harm.

The problems that come from heroin, or other opiate use, are directly related to it's illegal status. Namely:

Much higher prices - because it is illegal, prices are driven up by the artificial scarcity created, and the increased risk to those willing to provide it. This, in turn, creates crime on all sides of it. Users cannot hold down a job, even if their job pays enough to support a habit, due to irregular availability. Often stealing becomes the only reasonable alternative. One estimate I saw said that heroin is about forty times more expensive than it should be, if it weren't prohibited.

Crime is created because sellers have no legal way to enforce business obligations, and so resort to violence. We see these effects whenever something becomes prohibited. Remember Al Capone?

Poor health - the health of users suffers, not because of the drug itself, but because users do not have access to pure drugs, clean needles, honest information, or even health care itself, due both to fear of arrest and, because of the artificially high prices, the means to pay for health care, or to eat properly.

Arrest - one arrest, let alone a conviction, for drug possession can prevent people from obtaining decent employment, or financial aid for education. Obviously, this can only create more criminals, or individuals who cannot support themselves, or their families. Prohibition is responsible for the US having the largest prison population in the world. Almost a quarter of US prisoners are in for non-violent drug "offenses."

So, is heroin "bad?" I'll let you make that call. But clearly prohibition doesn't help the situation.
