how's it going? or how are you? It's a slightly more informal version of "?Cómo estás?"
Necro here thinks of it as. "Whats up?" Apparently not commonly used close to Mexico but is well understood. However it is not slightly informal. It's downright disrespectful to talk to a stranger in such a manner.
The response, normally, is "Bien, ?y tú?", which means "Well, and you?"

However, the word "qué" means "what" and the word "tal" means "such", so it literally means: "What such?", which of course is incomprehensible.
