It is a title given by the monarch of the UK (currently Elizabeth II) when she makes a man a knight in return for service to the crown or, more recently, the country. Honorary knighthoods can be given to foreigners (Bob Geldof has one), but they are not allowed to be called 'Sir'.

To be legally entitled to the rank of Sir, you must be male, a knight or baronet, and a citizen of the Commonwealth of Nations (one of the 16 sovereign states that recognize the head of England as their monarch). The monarch, currently Her Majesty Queen Elizabeth II, is the only one who can grant such an honour.

If you are knighted, but are not a citizen of the Commonwealth, you are not legally entitled to use 'Sir' before your name, though you may use the equivalent initials (such as KBE) following it.
