It generally refers to any un-paved portion of the roadway construction that is outside (to the edges of) the marked lanes of travel.
HOWEVER it can also refer to ANY portion of the roadway that is outside of the marked lanes of travel - WHETHER OR NOT it is hard surfaced. Paved shoulder are not meant, nor are they constructed, to routinely bear the weight of moving traffic.
Soft shoulder typically means dirt--mud-- untreated, unpaved, no gravel. Your wheel could enter mud; you could get stuck or slide. Heavy semis would likely get stuck if they pulled onto a soft shoulder.
The shoulder refers to the area of the road just outside the main road. A soft shoulder sign means that the shoulder is literally softer than the actual road.
The soft shoulder is a part of a road or highway that is unpaved and is usually covered in gavel. The soft shoulder sign informs drivers of the soft, unpaved ground along the edge of the road or highway.
It means that the shoulder of the road is soft and non-compacted, probably not hard-surfaced, and you could easily get stuck on the shoulder. In a situation like this, if you need to stop at the side of the road, make sure that your left wheels are still on the pavement.

Soft Top Hard Shoulder was created in 1993.
Most are placed in the soft shoulder on the side of the road. This is because it is so much easier to dig into soft dirt and gravel than solid pavement.

If a wheel goes onto a soft shoulder (off the edge of a road to a soft ground) a wheel will likely sink in, making it hard to get the vehicle out. If done at speed the vehicle may tip over.
