Medical diagnoses are based on symptoms, signs and the results of various diagnostic procedures.

Some of the medical diagnoses (e.g. myocardial infarction, pneumonia, ... ) are based on medical signs, which are "physical" parameters that can be destined objectively. Other medical diagnoses as tension headache, somatoform-pain disorder, fibromyalgia can not be destined objectively but subjectively only, because these diagnoses depend on symptoms only.

With psychiatric diagnoses it is the same. Psychiatric diagnoses are based on psychological symptoms, which are also called psycho-pathological phenomena. Because psychiatric diagnoses are based on symptoms (psychopathological phenomena) and not on medical signs (as electrocardiograph's findings, laboratory findings, MRT- pictures e.g. - which can be destined on an objective basis) the diagnosis itself is not objective but only subjective knowledge and valid within a convention (ICD-10 or DSM-IV e.g.)

A diagnosis of a mental disorder (psychiatric diagnosis) is based on abnormal psychological symptoms also called psychopathological phenomena.

Medical signs can be determined objectively on a physical basis, directly or indirectly.

Therefore many medical diagnoses can be determined objectively on a physical basis.

Examples of medical diagnoses which it is possible to prove on a physical basis are for example myocardial infarction through electrocardiagraphic and laboratory findings, pneumonia through X-ray findings.

Medical diagnoses based only on symptoms only, e.g. tension headache, somatoform-pain disorder, and fibromyalgia, cannot be proven on a physical basis; one can only carefully weigh in mind the symptom presence. Such diagnoses cannot be determined objectively, only subjectively.

The same is true for the diagnosis of a mental disorder.

Since diagnoses of mental disorders are based on psychological symptoms (psychopathological phenomena) and not on physical criteria such as laboratory findings, as outlined above, it follows that such a diagnosis cannot be proven on a physical basis, and, consequently, an objective diagnosis cannot be reached.

In the psychiatric diagnostic procedure the clinical picture of the symptoms also called the symptom complex is evaluated through comparison with the groupings of psychological elements which constitute psychiatric categories in a classification.

Thus, diagnoses based on symptoms (phenomena) alone, which are "mere ideas" can only be determined within a dogmaticconvention (gr. dogma=opinion).

Mere ideas in the sense of philosopher Immanuel Kant are ideas which it is not possible to prove on a physical basis ("am Probierstein der Erfahrung").
