Reincarnation is the belief that you come back as another person or animal. As far as I know there are no specifics as to whether you would remember your previous life or not.
Life after death refers to a number of beliefs. The most obvious being Heaven and Hell (or equivalent) as people's bodies' get left behind and their souls go to another place after dying. Other theories are things like ghosts/spirits/demons and things like that.
There are many more beliefs too which I don't know enough about to qualify writing about but I'm sure you could find quite easily on Google or Yahoo etc.

Many people belief in mythology, and they believe that God is reborn only as specific people. When somebody is born as an incarnation of God, they believe it is God appearing as this person on earth. But this is mythology. Some people believe in reincarnation, reincarnation is the belief that everybody who dies is reborn based on Karma. The body dies but the Mind and Ego, the ME, carries its Karma or its past actions and is reborn. Even this is only partly the Truth. Neither must we believe in incarnation nor reincarnation. We must go in quest of self-realization to realize that we are the Divine Soul, that we are all manifestations of God, that we are all Divine entities - that is the goal, to be liberated and unite with the Divine.
