In Theory; Approximately 73% dark energy, 23% dark matter, and 4% normal (baryonic) matter.

As far as we can tell now, the universe is made of energy, some of which appears in the form of matter.

Matter is composed of particles called atoms. Atoms form molecules; a molecule is the smallest part of a given material that still retains its chemical properties. Even atoms are composed of smaller particles like quarks and gluons.

It appears that all particles are waves at the same time.

Astronomers like to call all material made up of protons, neutrons and electrons "baryonic matter." Until about thirty years ago, astronomers thought that the universe was composed almost entirely of this "baryonic matter," ordinary atoms.

Hydrogen mostly, and some helium. The universe contains some heavier "trace elements"; we happen to think that they are important, because we ourselves are made of those "trace elements" like carbon, nitrogen and oxygen. Not to mention iron and potassium.

But the vast majority of the mass in the universe appears to be made of hydrogen.

Or, if you listen to the "dark matter" theorists, perhaps the majority of the mass in the universe isn't matter at all, or at least, not the way we would think about it.

Nothing - Seriously in percentage terms - nothing and the nothing bit is getting bigger.

Stars are made mostly of hydrogen.

There is also a lot of unseen matter and energy called dark matter and dark energy. We do not know exactly what they are, but there is a lot of it, whatever it is.

The vast majority of all the matter in the universe is Hydrogen, the simplest element. The next-most-common element is Helium.

Anything more complex than hydrogen or helium is the result of nuclear fusion in the cores of stars. When a massive star dies in a supernova explosion, much of the helium "ash" in the star is re-fused into heavier elements, and much of that is blown off into space. This is the source of all the heavy elements such as iron, gold, and lead - and everything else - that later collapses to form planets.

Good question. When I was growing up, the answer would have been "interstellar clouds of dust and gas", but more and more these days, the answer is looking like "supermassive black holes in the centers of many large galaxies".

It is certainly possible that this answer, too, will turn out to be incorrect. We're still learning loads of stuff about the universe every day, and sometimes we don't entirely understand the parts that we do know.
