John F. Kennedy earned the Purple Heart during WWII when his boat, PT-109, was rammed by a Japanese destroyer while on night patrol. He suffered a back injury that worsened a previous condition.

Presidential candidates John McCain and John Kerry have also received the award for being injured in battle. James Monroe and Rutherford Hayes were both wounded in battle but the award was not in existence until 1917.
