In the Middle Ages, marriage was considered primarily a private matter, between couples. While the Church had rules about it, these had more to do with such things as divorce and the sin of adultery than with the wedding. Marriage in those days did not have to happen in a church, did not require a priest, and did not even require witnesses. It did require vows, and the fact that a wedding had happened meant that the vows had happened. It was a verbal contract between two people, undertaken at a time when a person's worth was that person's word. A marriage could be registered with the local church, and while this was customary in some places, it was not required by the Church.
There is a lot of misunderstanding about life in medieval Europe. Common people did not normally have arranged marriages except where local customs had family involvement in marriage. Common people did not marry young, but after the bride had put together her dowry and the groom had established an income. This usually happened when the couple was in their 20's.
