The answer is none, the only astronauts who have been to the moon got there in various Lunar Modules, all of which were launched on Apollo rockets.
On the 12th of April 1981, 20 years after Yuri Gagarine became the first man in space, Commander John Young and Pilot Robert Crippen became the first astronauts to fly the space shuttle in space. They returned safely two days later, the 14th of April.

Columbia was the first shuttle to enter orbit. This was the same shuttle that was destroyed during re-entry in January 2003, with the loss of seven astronauts.
