Its the same theory as in a standard trany. A 4 speed standard trany makes the motor rev at a certain RPM when the motor is not under great load. If you add a 5th gear to drop the rpms of the engine by 30-40 percent at a lower rpm the car will achieve a much better fuel efficiency.
