in the last few years, since soon after the fall of the taliban, Afghanistan has come to be the major supplier of opium worldwide--most estimates put it at about 90% of the world supply. Since opium is the main ingredient of heroin, it in some sense can be said to generally come from Afghanistan. There is an essential chemical component in the conversion of morphine to heroin (that's three steps in all: opium becomes morphine becomes heroin) which is an uncommon enough chemical that the united states drug enforcement agency is able to maintain very strict monitering of its use and manufacture, effectively making the manufacture of heroin impossible in the US on any scale. this means, assuming your hypothetical dealer is in the united states, that the heroin they've got has likely been manufactured in a third country where the necessary chemical is easier to acquire or manufacture without interference from law enforcement. the necessary scale of an operation involving this much travel, necessary that is for the thing to be profitable, would almost inevitably require some kind of organized crime. often, several different "criminal organizations" will profit or play a role in the sale of a single batch of heroin--so a terrorist organization might be handling the large scale export and conversion to morphine of the crops of individual opium poppy growers in Afghanistan, while something like the Mexican mafia could be handling the transport of morphine to Mexico, its conversion to heroin, its smuggle across the American border, and its distribution to at least the first rung of dealers in the U.S. there are at least a few steps down from those top dealers to any dealer likely to be encountered by a user--each step down generally involves the addition of more "fillers" with which the drug is cut in order to garner a profit for the seller. so the dollar-per-ounce of stuff may remain somewhat constant all the way down the chain, but the purity will continue to diminish. depending on the criminal organization handling the particular territory serviced by your dealer, they may maintain more or less control of the street-level dealers. some organizations keep assiduous track of the product all the way down the chain, and may even go so far as to maintain a second business of knowing who their users are, and making that useful to themselves when necessary. other organizations are less comprehensive, and street level dealers of their product are truly functioning independent contractors or small-business people. higher-ups will try to keep lower-downs from knowing much about the tiers above them, in order to insulate the organization from the law. this mean that some dealers are likely to be completely ignorant of the source of their own supply beyond their direct connection.

This controls involuntary responses like heart rate, respiration, fear & stress response, auditory & sight perception, hunger, metabolism, sleep cycle etc. When someone is chronically using heroin the brain adjusts & compensates for the drugs effects. When the drug begins to leave the body the brain can no longer regulate the simple involuntary responses we take for granted. This causes an uncontrollable violent physical reaction. From a psychological point of view the brain actually believes it is dying. That is why severe addicts do horrible things to support their habit.

It depends on the area. People in or near a large city center such as Chicago go to open air drug markets in areas known for it. It is usually a high crime, lower class neighborhood. In such areas, there are usually individuals off the main larger streets that hang out on smaller streets nearby. In the country or suburbs, it is usually a who you know situation. It is common for individuals to go to the city and bring large amount out to sell to those not in the city. All this is of course Illegal.
