I just worked on a 1997 Ram Xcab 4x4 last night. 'm not sure if it is the same but- The blend door was actuated by a cable and was located on top of the heater box assembly just beside the actual heat coil. The dash must be removed before it can be accessed. It is really not too bad. I removed the heat coil and checked/cleaned it and then reassembled everything in about 3 hours. The blend door didn't have any problems on my truck. The actual problem was in the heat coil. It was not installed correctly in the box.

The heater core may be plugged. With the engine running and fully warmed up, feel both of the heater hoses with your hands. If one hose is hot and the other is luke warm or cold, the heater is plugged in which case you can try flushing the heater core. If both hoses are hot, the temp blend door acuator behind the dash assembly is broken. If both hoses are cold, replace the thermostat again and be sure the cooling system is full.
Check the heater hoses on the right side of the engine. If one hose is hot and the other is not , you might have a blocked heater core. Or the blend door actuator might not be operating correctly.
The temperature blend door may be at fault.
Behind the dash, connected to the blend knob and runs down to the HVAC housing at the front passenger of the cabin, also behind the dash. The blend door is inside the HVAC housing adjacent to the heater core.
The heater flap, or blend door, is difficult to replace with OEM parts supplied from Ford. There is an aftermarket kit that makes the job and cost significantly easier at heatertreater.net or on the Ebay listing.
You have a faulty temperature blend door actuator located directly behind the radio. Remove the radio and remove the blend door actuator held to the heater case with three 8mm screws, and replace it.
under hood, disconnect coolant hoses where they go into firewall in cab, remove glove box.then remove screws that hold platic cover behind glovebox this will give u access to heater core undo screws remove heater core install heatercore, rplace plastic cover,replace glove box under hood re attach heater hoses job is done,NOTE u do not need to drain engine coolant you can position heater hoses where they will not drain 1996 & older this is right 1997 new generation you have to drop steering column, set dash back then disassemble box. Good idea to replace blend door at same time they are giving trouble breaking at acuator motor
Assuming you want it to blow hot, it could be low on coolant, a stuck open thermostat, a plugged heater core or a faulty temp blend door.
On the bottom of the heater/ac assembly in front of the passenger compartment . YOU KNOW BEHIND THE DASH .
The problem is the blend door, not the heater core. Any mechanic worth anything should have checked and replaced the blend doors while it was apart to replace the heater core. Check heatertreater.net for diagnostic information and some real help.
