It's unclear to me whether the episode is actually *based* on "Telek", but "Plato's Stepchildren" shares a very similar plot to that story.
The Moon Moth - by Jack Vance
Some Law and Order episodes were inspired by true stories, however none are 'based' on true stories.
None.. You are referring to an episode of "1000 Ways To Die". Bad Max. In the episode, It is admitted that the accident did not happen to an actor. But it is based on a true story.
Yes. I believe there were 4 stories, not including the "wraparound" story. All of the stories except the first one where the guy was a racist were either direct remakes of an episode, or based very closely on an episode from the original series from the 60s.

All of the Law and Order series stories are fictional. However, their theme, 'Ripped from the headlines' is often true in that they take a real story from the news and make their own, fictional version of it. I often recognize the real story that an episode came from. The stories are often very much the same in the beginning of the Law and Order episode, but always takes twists and turns that end the story unrelated to the real one.
They performed West Side Story in the 5th episode of season 3 called ''The First Time.'
